Self Pay 

First visit:  $200 for 55 minutes.

Follow-up visits:  $180 for 55 minutes.

Payment due at time of visit.

 

Insurance

I am an “in-network” provider with these insurance companies:

  • Cigna

  • Regence BlueCross BlueShield of Oregon

  • Medicare

  • Pacific Source - Commercial

  • Pacific Source Community Solutions / OHP / Medicaid 

 

Even if I am “in-network” with your insurance company it does not mean nutrition counseling is a covered benefit for you.  This depends on your specific plan and your medical diagnosis.  To understand your benefits, call your insurance company and ask these questions:

  • Do I have covered benefits for Medical Nutrition Therapy:  CPT codes 97802 and 97803?

  • If yes, what is the limit, if any, to the number of visits each year?

  • Do I have a deductible to meet before insurance pays?

  • Do I have a co-pay or co-insurance for these visits?
